The goal of the program is to divert clients away from the state hospital system and into intensive county-based treatment programs where they will provide housing and wrap-around services to address the client's immediate mental health needs and connect them with lifelong services even after their criminal case is resolved.
The Clinical Social Worker II will be responsible for interviewing and screening potential clients for eligibility, obtaining necessary releases and medical records, interviewing family members as necessary, and scheduling evaluations and risk assessments with the program's forensic psychologist. Once a client has been accepted into the program, the social worker will assist the client in applying for MediCal and Social Security and coordinating with the treatment provider for the client's release into the program.
